通常在兩種情況下會建議患者考慮這項手術，第一是牙齒因為外傷導致牙冠斷裂、蛀牙範圍延伸到牙齦的下方、或是先天臨床牙冠過短，這將會使假牙重建所需的固持力不足，導致無法製作高品質的贗復假牙，此時就會建議進行牙冠增長術；第二種是美觀上的考量，不協調不對稱的牙齦邊緣、或是當病人微笑時露出的牙齦太多，會影響笑容的美感，此時也可以利用牙冠增長術改善美觀，如下圖比較。

如果病人曾經因為牙周病、長期缺牙、刷牙習慣不佳或是年齡漸長等因素，使角化牙齦有比較明顯的萎縮，甚至導致牙根裸露的狀況時，除了有美觀上的問題，也可能讓牙齒變得更加敏感而不適，或是因為清潔不易導致牙根蛀牙或是牙周病的不斷復發；如果在植牙區的角化牙齦不足，也很有可能因為清潔效果不佳導致植體周圍炎，因此不論是天然的牙齒或是不會蛀牙的人工植牙，能好好清潔的健康軟組織都是必不可少的。
