其實不像一般人所誤會的「醫師喜歡拔牙」，不如說恰好相反，如果可以的話，治療方向都會盡量以保留原生牙為主，畢竟大部分假牙種類其實都多多少少會有缺點，終究比不上真牙，所以可以的話醫師當然不想拔掉。

但是如果很少回診，或是口腔清潔不落實， 第二大臼齒蛀牙 又剛好從比較刁鑽的位置蛀進去，那可能就會比較麻煩，通常醫師會先根管治療試試，看能不能透過這樣就解決問題，大部分這樣就可以解決問題，但是通常會建議根管治療完牙齒要套上牙套，避免後續牙齒裂掉，因為根管治療完的牙齒會比較脆弱，如果牙齒裂掉、失去咀嚼功能，那就真的必須拔除了。

因為位置不好將器具探入，更沒有可以觀察的視野，首先通常會透過X光來觀察蛀牙的狀況，觀察完後才會開始修補，如果不嚴重不需要根管治療，那就會先在咬合面打洞，接著深入該區填補，但也不需要擔心，如果有需要醫師會上局部麻醉， 不會有多少疼痛感，在修補完後醫師也會補上鑿開的洞，將其恢復原樣 。
